All 2025 Lifeline Outreach Grant Project Reports must be completed in the PSC Grant System.
Full project reporting requires the submission of three reports over the one-year grant performance period (2 Interim Project Status Reports and 1 Final Project Summary Report).
Interim Project Status Reports – each consists of two documents:
- 2025 Lifeline Outreach Interim Status Report Event Log - 2025 LLG Events excel file to be downloaded in advance, completed as a separate file, and uploaded to the PSC Grant System in Question 1 of each Interim Status Report.
- 2025 Lifeline Outreach Interim Status Report - Report Sample for View Only. The reports can only be filled out in the PSC Grants System.

Reporting Schedule
Interim Status Reports are due biannually, resulting in 2 reporting periods. Each interim project status report covers activities from the preceding 6 months:

S1 – Due January 15, 2026 (7/1/25 – 12/31/25)

S2 – Due July 15, 2026 (1/1/26 – 6/30/26)
Final Project Summary Report – consists of a single document summarizing the project.
