VoIP Provider Registration Requirements

Under Wisconsin Statutes, VoIP providers are required to register with the Commission, and to file, annually, revenue information through the annual questionnaire. VoIP providers, like all Wisconsin providers in all statutory categories, are subject to assessment for the Wisconsin universal service fund and for Commission expenses pursuant to Wis. Stat. § 196.85.

When you launch the application, make note of the application number. You can edit an unsubmitted application using that number.

VoIP providers are required to upload a PDF copy of the company’s articles of incorporation (or equivalent) and, if an out-of-state company, a copy of the company’s registration with the Wis. Dept. of Financial Institutions.
Complete applications are normally processed within a day or two of receipt. The provider will be notified of completion via email. That email will include the company’s utility identification number.
