An incident is defined in 49 CFR §191.3 as a release of natural gas from an operator's system resulting in a death, a serious injury, significant property damage, significant gas loss, or a significant emergency event.

It includes investigations on possible incidents even if they do not meet the definition of an incident.

PHMSA Incident Data

PHMSA also collects and shares incident data going back 20 years which can be found on their Pipeline Incident Trends page.
