The 2006 Web-based Telecommunications Annual Report Program is used by telecommunications
resellers (RES), cable television telecommunications service providers (CTV), commercial
mobile radio service providers (CMR), interexchange carriers (ARW), and competitive local
exchange carriers (OTH) with no activity in Wisconsin. The statutory due date
for these reports is April 1, 2007. Because April 1, 2007, is on a Sunday, the
reports are due Monday, April 2, 2007.

Program and Features
To use these web-based programs, users will need a password (see below).
Users must also have Microsoft Internet Explorer (IE) 5.0 (or higher) installed on
their computer; Netscape browser is not supported by these programs. Users
also need Adobe Acrobat Reader 5.0 (or higher) installed on their computer to view Help
files and the Rules for Reporting. Free downloads are available at
http://www.microsoft.com
and
http://www.adobe.com, respectively.
Selected identification and contact information has been prepopulated from the prior
year’s filed annual report; edit as necessary.
Process for Completing and Filing Annual Reports:
- Locate Password Letter: Locate your letter from the
Public Service Commission (PSC) that identifies the Annual Report Filing password
for the utility. The letter, dated February 9, 2004, or later, was addressed
to the primary utility address in the Commission's records. Passwords are
permanent and should be used year-after-year.

If you misplaced your password letter, there are ways to receive it again.
If you were a contact on the most recently-filed PSC annual report, you may
request your annual report password on-line by following the instructions
at that site. Other users will have to request a password letter from the
Telecommunications Division by contacting Cindy Gilles at (608) 267-2893 or
Cindy.Gilles@psc.state.wi.us.
- Access Annual Report: On the PSC home web page, select the
Annual Report button on the left-hand side.
Scroll to Annual Report Programs and select Telecommunications (2006).
Select the annual report type to be filed. CTV annual reports will be filed using
a connection secured by SSL (Secure Sockets Layer) technology. The secure connection
will encrypt all information before sending it to the PSC.

- Logon: A logon page will appear prompting the user for name, email
address, phone number, utility name and annual report filing password. The
user will acknowledge he or she is authorized to file the annual report, will
acknowledge the statutes pertaining to the annual report, and then will click
"continue."
- Time Limit: If the utility and password match, the annual report
screen will appear and the preparer will have 30 minutes to submit the annual report
information. For security reasons, after 30 minutes of inactivity,
the program will clear the data and the preparer will have to start over. It
is recommended that the preparer print out the form, gather the required information,
and access the program a second time to comply with this time requirement.
- Navigation: The preparer should use the Tab/Shift-Tab key to navigate within
the form. All dollar amounts are reported to the nearest thousand dollars
(example: $130,000 should be reported as $130.) Do not type commas when entering
numbers. Once the information is entered, preparer should print a copy of the
annual report before electronically submitting it.
- Public Information: The ARW, CMR, RES, and OTH No Activity in
Wisconsin annual reports provide no confidential treatment of annual report information;
the CTV annual report provides provisional confidential treatment for selected items.
The respective "Information Regarding Confidential Filings" document shown
below describes which items, if any, qualify for confidential treatment for your utility's
2006 annual report. If you desire additional provisional confidential treatment
above that provided in your annual report form, do not electronically file the annual report
using the Web-based application. Instead, contact Christopher Larson by phone at
(608) 267-9508 or by e-mail at
Christopher.larson@psc.state.wi.us for further instructions.
- Assessable Revenues:Please pay particular attention to reporting assessable
revenues correctly. The Commission relies on this information to compute and bill assessments for
remainder, telephone relay, and Universal Service Fund programs. Requests for adjustments and
refunds of assessments may involve a hearing, and objections received after the statutory appeal
period may be denied.
- Missing Data: If there is missing data, the user will be prompted in red
to fix the problems. (Please note that the user may be required to scroll up or down to
view all error messages!) If the report is complete, a confirmation page will appear and
the preparer should print the confirmation page as proof of filing.
NOTE: If the preparer is filing a RES report and the reseller has
less than $200,000 in assessable revenues for remainder and telephone relay
assessment purposes, the confirmation page will notify the preparer to
send a check for $100 to the PSC if the reseller wishes to continue its
intrastate certification.

telecommunications providers who wish to request confidential handling of portions of
their annual reports.

Refiling an Annual Report:
The user must start from scratch and repeat the entire filing process listed above. Once a report is filed,
the data is not available for editing. When the user prepares a new report, selected identification and contact
information will be prepopulated from the prior year’s filed annual report (not the current year's previously-filed
annual report); edit as necessary.
