Notes:
1.	The Wisconsin State Telecommunications Association files an access tariff for most Incumbent Local Exchange Carriers (ILEC) in Wisconsin. Exceptions include SBC, Verizon and CenturyTel. The rules and regulations governing such service are found in: Wisconsin State Telecommunications Association. The list of issuing carriers begins on sheet two of this document. See the tariff for each individual company to find its access rates.
2.	TDS Telecom files a general local exchange tariff for its Incumbent Local Exchange Carriers (ILEC) subsidiaries in Wisconsin, (all TDS companies except Mid-Plains, Inc.). 3.	The tariff for SBC is found under Wisconsin Bell, Inc., Choice One Communications is found under US-Xchange of Wisconsin
